In my office we need to collect responses to a 20 question survey from over 200 respondants. An online survey tool is not an option. Obviously our challenges are:

• distributing the survey
• collecting valid data (e.g. dates in a consistent format, required fields, etc)
• an organized means of collecting the responses
• analyzing the responses through calculations like median, averages, sums, etc.

Our attempt last year to do this failed miserably when we tried sending Word documents with form fields and instructions out to the respondants, asking that they email them back.

My question then is, what would you recommend we use for this task?
